At Premier Vein and Body we strive to make our services affordable for every patient. With a variety of ways to pay, which may be used individually or combined according to your wishes, there should be nothing standing between you and your desire to “Achieve Your Desires.”

All payments for cosmetic services are due in full prior to the commencement of treatment. We provide the following payment options:
- Cash or Check: Personal check, cashier’s check, or cash
- American Express

Think of CareCredit as your own health, wellness and beauty credit card. Whether it’s plastic surgery, facial rejuvenation or a skin care visit, you shouldn’t have to worry about how to get the procedures you want. That’s why we’re pleased to accept the CareCredit health, wellness and beauty credit card. CareCredit lets you say “Yes” to recommended surgical and non-surgical cosmetic procedures, and pay for them in convenient monthly payments that fit your financial situation.
Varicose veins are considered to be medically necessary, and treatment is, generally, covered by medical insurance if the patient has developed complications or has activity-impairing symptoms of discomfort, and if ultrasound evaluation documents insufficiency or reflux (backwards flow in the vein). In this case, insurance companies consider treatment for varicose veins to be medically necessary and, typically, will provide coverage. Some insurance companies require a trial of compression stockings before approving other treatments.
Our staff will do everything possible to maximize your benefits and make the insurance process as smooth as possible. Premier Vein & Body participates with the following insurance plans:

Please note that a patient can still obtain insurance coverage for a vein procedure from Dr. Schwartz even if we are out-of-network with one’s insurance as long as they have a PPO policy.

For cosmetic, aesthetic and non-surgical body contouring treatment sessions, such as Sclerotherapy, SculpSure®, CoolSculpting®, Venus Freeze™ and Viva, miraDry®, Laser/Skin, and BOTOX® Cosmetic/Fillers cancellations:
We appreciate 48 hours (2 business days) notice of cancellation so that we may work with another patient if you are unable to keep the appointment for your treatment session. Treatment appointments cancelled with less than 48 hours (2 business days) notice or No-Shows will incur a $100 fee.
For Laser Ablation vein surgery cancellations:
We appreciate 2 weeks’ notice of cancellation for vein surgeries so that we may work with another patient during your scheduled time. Surgery cancelled with less than 2 weeks’ notice will incur a $200 fee. Surgery cancelled with less than 48 hours’ (2 business days) notice or No-Shows will incur a $500 fee.
For Liposuction and Surgical Body Contouring cancellations:
We appreciate 2 weeks’ notice of cancellation for lipo or body contouring surgeries so that we may work with another patient during your scheduled time. Surgery cancelled with less than 2 weeks’ notice will incur a $500 fee on your credit card. Surgery cancelled with less than 48 hours’ (2 business days) notice or No-Shows will forfeit the deposit paid at time of scheduling.
